How Long Does Tongkat Ali Take to Work? The Week-by-Week Timeline

Tongkat ali runs on more than one clock. Fatigue can ease in two weeks, testosterone follows in about a month at 200 mg, and sexual function takes about three. Here is what to expect, and how to tell if yours is working.

Key takeaways

  • Week 2: fatigue and aging-male symptoms can already ease. In older men with low testosterone, symptom scores beat placebo at the first check.
  • Week 4: testosterone moves, at 200 mg a day. At 100 mg it took 12 weeks.
  • Week 12: libido, erections and sperm quality improve in the trials. This is your decision point.
  • Each goal has its own checkpoint. Judge energy at week 4, strength between weeks 5 and 12, sex drive at week 12, and bloodwork at week 12 (week 4 to 8 on 200 mg or more).
  • Fastest responders: men over 40, men with low or low-normal testosterone, and stressed men.

Most men take tongkat ali for five days, feel nothing, and call it a scam.

That is like planting a tree and checking for fruit on Friday.

Tongkat ali (Eurycoma longifolia) is not a stimulant. It works on the system that makes testosterone and on the stress hormone that suppresses it, and that system runs on a clock of weeks. Actually, several clocks: the human trials show different benefits arriving at different times.

The good news is that those clocks are well mapped. Tongkat ali has randomized trials running from two weeks to six months. Put them side by side and a clear schedule appears.

12 weeks

When libido, erectile function and sperm quality improved against placebo in the trials. Energy and testosterone come earlier.

Ismail et al., 2012; Chinnappan et al., 2021

A 2022 meta-analysis of randomized trials found a significant rise in total testosterone with tongkat ali, in both healthy men and men with low testosterone (Leisegang et al., 2022). A 2024 systematic review of 27 proposed testosterone boosters rated it "possibly effective" for both groups, one of very few to make that list (Morgado et al., 2024).

The useful question is not whether it works. It is what to expect, and when.

Chart of when each tongkat ali benefit first appeared in human trials: fatigue and symptoms at week 2, testosterone at week 4 on 200 mg or week 12 on 100 mg, strength from weeks 5 to 12, libido and erections at week 12, and the largest gains with training at 6 months
When each result first beat placebo or baseline in the trials: Chinnappan 2021, Talbott 2013, Henkel 2014, Ismail 2012, Leitão 2021.

The tongkat ali timeline, stage by stage

01 Week 2: you may feel it before the bloodwork shows it

The earliest change is in how men feel. In a 12-week trial of men aged 50 to 70 with low testosterone, aging-male symptom scores and fatigue scores were already better than placebo at the first check, week 2, at both 100 mg and 200 mg a day (Chinnappan et al., 2021). DHEA, a precursor hormone, was also climbing from week 2.

This is usually what men describe as something being different. Better mornings. Less afternoon drag. Stress that lands a little softer.

Hormones can move this fast too, at high doses. In young men taking 600 mg a day, testosterone and free testosterone rose against placebo in just two weeks (Chan et al., 2021).

Honest scope

The week-2 changes in older men were symptom and fatigue questionnaires, not testosterone against placebo. Treat week 2 as an early signal, not the payoff.

02 Weeks 2 to 4: cortisol drops and testosterone follows

This is where the strongest hormone data sits.

In moderately stressed adults, four weeks of tongkat ali cut cortisol exposure by 16% and raised salivary testosterone by 37% against placebo. Tension, anger and confusion scores all fell (Talbott et al., 2013).

That matters because cortisol and testosterone pull in opposite directions. Chronic stress raises one and suppresses the other. Tongkat ali appears to work partly by taking the brake off.

In the Chinnappan trial, total testosterone at 200 mg beat placebo by week 4 and stayed ahead through weeks 8 and 12. In men with late-onset hypogonadism, one month of 200 mg a day moved the share of men with normal testosterone from 35.5% to 90.8% (Tambi et al., 2012).

Honest scope

The Tambi study had no placebo group. The controlled trials still point the same way: at 200 mg, week 4 is where testosterone changes become reliable.

03 Weeks 5 to 12: strength builds with training

Once the hormones shift, the body starts spending them.

Active men and women aged 57 to 72 took 400 mg a day for five weeks. Total and free testosterone rose, and so did grip strength (Henkel et al., 2014). In the Chinnappan trial, muscle strength was up by week 12.

Honest scope

The Henkel study was a small pilot with no placebo group. The strength gains fit the hormone data, but they are the least proven part of the timeline.

04 Week 12: libido and sexual function

Sexual effects are the slowest to arrive, and the best documented once they do.

In a 12-week randomized trial of 109 men aged 30 to 55, 300 mg a day improved erectile function scores against placebo. Libido rose 14% by week 12, sperm motility 44.4%, and semen volume 18.2% (Ismail et al., 2012).

A second 12-week trial in men aged 40 to 65 found better erection hardness, intercourse performance and aging-male symptom scores than placebo (Udani et al., 2014).

Why so slow? Sexual function sits downstream of everything above: hormones, energy, stress, blood flow. It is the last domino.

Honest scope

The Udani trial combined tongkat ali with a second herb, Polygonum minus, and only 26 men finished it. The Ismail trial is the cleaner test of tongkat ali alone.

05 Month 6: training multiplies it

The longest trial ran six months in men aged around 47 with androgen deficiency. It tested 200 mg of tongkat ali, a training program, and both together.

The combination won. Men who trained and took tongkat ali had the largest improvement in erectile function and a rise in total testosterone (Leitão et al., 2021). The same group gained the most leg strength (Leitão et al., 2021).

Tongkat ali multiplies training. It does not replace it. The best results in the longest trial came from men who took it and trained three times a week.

The trials behind the timeline

Study Who Daily dose When it showed up What changed
Chinnappan 2021 (symptoms) Men 50 to 70, low T 100 or 200 mg 2 weeks Fatigue and symptom scores
Chan 2021 Men, ~24 yrs 600 mg 2 weeks Total and free testosterone
Talbott 2013 Stressed adults 200 mg 4 weeks Cortisol down 16%, testosterone up 37%
Chinnappan 2021 (testosterone) Men 50 to 70, low T 200 mg 4 weeks Total testosterone vs placebo
Henkel 2014 Active seniors 400 mg 5 weeks Testosterone, grip strength
Ismail 2012 Men 30 to 55 300 mg 12 weeks Erectile function, libido, sperm motility
Leitão 2021 Men ~47, androgen deficiency 200 mg 6 months Erectile function, testosterone (with training)

Your dose sets the clock

Only one trial compared doses directly, and the difference was in timing, not just size. At 200 mg a day, total testosterone beat placebo from week 4. At 100 mg it got there at week 12. Fatigue and symptom scores improved from week 2 at both doses (Chinnappan et al., 2021).

So read any timeline, including this one, against your dose. Trials used 100 to 600 mg a day, most often 200 to 400 mg, nearly always a standardized water extract of the root. Raw root powder is not the same thing, and the trial clocks do not transfer to it.

Does this timeline apply to you?

The trials point to three groups who respond fastest and most:

  • Men over 40. Most of the trials above enrolled men over 40.
  • Men with low or low-normal testosterone. The meta-analysis confirmed the effect in the hypogonadism subgroup specifically.
  • Stressed men. If cortisol is the brake, taking it off matters most when it is pressed hardest.

If your testosterone is already solidly normal, expect a smaller change. Young men on a high dose did see a rise (Chan, 2021), but the evidence is thinnest for them.

How to tell it is working

Do not judge everything on one day. Give each goal its own checkpoint, and write down a baseline before you start. If your bottle is a one-month supply, have 12 weeks on hand before day one: the slowest results arrive after the first bottle runs out.

Your goal Judge it at What to track
Energy, fatigue, stress Week 4 (first changes from week 2) Energy and mood, scored 1 to 10 weekly
Strength Weeks 5 to 12, with training Main lifts, every session
Libido and erections Week 12 Morning erections and desire, weekly
Testosterone on a blood test Week 12 (week 4 to 8 on 200 mg or more) Morning blood test, before day one and at the checkpoint

Track the same way every time. Testosterone changes through the day, so test in the morning both times, ideally at the same lab.

Compare against your baseline, not your memory. Week-by-week changes are small. A two-line weekly note shows a trend you will not feel day to day.

If nothing has changed

At week 4, nothing at all? Check the basics first. The trials used a standardized root extract, taken every day, at 100 to 600 mg. Your label should say root extract with a stated standardization (usually a percentage of eurycomanone), not root powder. Skipped days or an unknown dose break the comparison. If the basics check out, keep going to week 12, your decision point.

Feel better but no lab change yet? That matches the trial pattern at lower doses: symptoms and fatigue improved before testosterone did. Keep going to week 12 before you decide.

Not training? The longest trial found the biggest gains in men who combined tongkat ali with three sessions a week (Leitão et al., 2021). Sleep and stress count too: cortisol is the brake tongkat ali works against.

At week 12, nothing on your log has moved? Then the test has given its answer: by week 12, testosterone had moved at both doses in the trials. No bloodwork? A flat log at week 12 is your signal to stop. With bloodwork: if your levels were already solidly normal, a small change was the expected result, and you can stop. If they were low and nothing moved, take the numbers to your doctor.

Where Alpha fits

On Alpha you are on the 100 mg clock. Each 3-capsule serving of Notable Alpha™ has 100 mg of tongkat ali root extract standardized to 10% eurycomanone.

That is the lower dose in the Chinnappan trial: the arm where fatigue and symptom scores improved from week 2 and total testosterone beat placebo at week 12. So judge energy at week 4, and everything else at week 12.

It does not lean on tongkat alone. Testosterone production also depends on vitamin D, minerals and how well you handle stress, so the same serving carries nine other ingredients that work on that system from other directions, including shilajit, Testncrease® fenugreek with black musli, fadogia, cistanche, boron, and vitamins D3 and K2.

If you are weighing tongkat against fenugreek, the other big name in this space, we compared them goal by goal in Tongkat Ali vs Fenugreek.

Your first order is covered by a 90-day money-back guarantee, long enough to run the full 12-week test, and we offer blood panels if you want to track your numbers.

FAQ

Can you feel tongkat ali on the first day?

Some men report a lift in mood or drive within days, but no trial measured that early. The earliest measured changes came at two weeks: fatigue and symptom scores in older men, and testosterone in young men on a high dose.

Should you cycle tongkat ali?

There is no human evidence that cycling helps. Trials dosed daily for up to six months without breaks, and the effects kept building over time.

Will tongkat ali show up on a sports drug test?

In recreational athletes, 400 mg a day for six weeks did not change the urinary testosterone to epitestosterone ratio that anti-doping tests use (Chen et al., 2014).
